A man accused of making telephone threats and causing fear of violence refused to answer to his name at Worcester Crown Court.
Peter Collins also declined to enter a plea when the charges were put to him.
Judge Marten Coates ordered not guilty pleas be entered on charges of harassment, having a knife as an offensive weapon at Kidder-minster Hospital, and common assault.
The judge said he was satisfied that Collins, whose address was given as Flint Close, Kidderm-inster, when he was committed for trial by Kidderminster magistrates, was fit to plead and remanded him in custody for a trial date to be fixed.
