People in Worcestershire can take their health into their own hands using an online tool, hosted by Diabetes UK, to calculate their risk of developing type 2 diabetes.

The Diabetes UK ‘know your risk’ tool, scores people on their risk of developing type 2 diabetes. If your risk is moderate or high, you can refer yourself to a local scheme that is part of the Healthier You: NHS Diabetes Prevention Programme and get easy access to free health and lifestyle coaching.

Everyone participating in the programme will have a choice between app based one-to-one personal coaching and group sessions. Access to the programme is completely free for users and available without having to go through a healthcare professional.

Throughout Worcestershire, this service is provided by personalised digital health coaching provider, Liva Healthcare, and in-person educational support from non-profit health provider, Living Well Taking Control.

William Bachop, pre-diabetic and user on the app-based programme says, “My lifestyle has changed dramatically since I joined the programme. I’ve reduced my HbA1c levels from 43-38 mmol/mol and had a weight loss of 9.5kg. I’m eating less sugar and more fruits and vegetables. My wife is also participating in my new diet. The best thing about having my own personal health coach is that I have someone who understands my specific needs.”
